The only difference is that you spawn on an island in the middle of nowhere, featuring a chest with items, and a tree. The basic point of this is to try and survive using your wits and a few cobblestone generators :P
You can respawn during Skyblock survival if you die, but in general accordance to Minecraft, you will lose your things and will have to start over again.
